Surrogate Application
Share your story, health history, and preferences so our team and your clinic can understand your situation and see whether surrogacy is a safe fit for you.
This application is usually the next step after you've explored our information, reviewed requirements, or spoken with our team. It's where you can share more detail about your pregnancies, health, lifestyle, and preferences so we can understand your situation and support you well.
Completing this application does not create a contract, guarantee acceptance, or obligate you to move forward. It's a screening and planning tool, not a test. You are welcome to take your time, skip questions that don't apply, and ask for help if anything feels confusing or sensitive.
Our goal is to make sure surrogacy is medically safe, emotionally sustainable, and realistic for your life and family.
Founder-led by former surrogate and five-time egg donor Diana Olmeda since 1998 from San Diego, California, with 28 years of continuous agency experience in 2026.

What Is the Surrogate Application?
Many women wonder what it really means to "apply" to be a surrogate and worry it might commit them to something they're not ready for. This section explains the purpose of the form so you can decide when and how to proceed.
This application is:
- A structured way to share your background, pregnancies, health, and support system.
- A tool for our team and the fertility clinic to understand whether surrogacy is a safe option for you.
- A starting point for thoughtful conversations about matching, compensation, and support.
This application is not:
- A contract or legal agreement.
- A promise that you will be accepted or matched.
- A commitment to move forward if you decide surrogacy isn't right for you.
What We Ask You About
The application covers several areas so our team can understand your situation and support you appropriately. Not every question will apply to everyone, and some answers may be "I'm not sure yet." That's okay. Sensitive topics can also be discussed directly with your coordinator if you prefer.
Key topics included:
- Basic information: contact details, where you live, and how you prefer to communicate.
- Family and living situation: who lives in your home, relationship status, and who supports you.
- Pregnancy and delivery history: prior pregnancies, deliveries, and any complications your doctors should be aware of.
- Medical background: health conditions, medications, and surgeries that might affect pregnancy.
- Lifestyle and work: schedule, physical demands, childcare responsibilities, and typical routines.
- Insurance and practical planning: whether you have health insurance and any context that helps us plan safely.
- Preferences: types of families you feel drawn to support, communication style, and how involved you'd like to be.
- Emotional support: anything we should know so we can set you up with the right support during the process.
If you're unsure how to answer a question, it's okay to leave it blank and talk it through later. Nothing in this application replaces medical or legal advice from your own professionals.
Estimated time: many surrogates take about 15–25 minutes, depending on how much detail they share. You can save your progress and return later.
How We Use Your Information
Trust is central to surrogacy. You're sharing personal information, and you deserve to know how it's used and protected.
- Your responses help us and the IVF clinic assess whether surrogacy is likely to be safe and appropriate for you.
- Information guides next-step conversations about matching, compensation, and support.
- Your preferences help shape the kind of intended parents we look for when it's time to match.
- We do not sell your information.
- Your application is not shared with third parties without your knowledge and direction.
You're in control.
This application is informational and non-binding. You can pause, skip questions, and come back later.
When You're Ready to Begin
Some women are ready to complete the application right away. Others prefer a smaller step first. Wherever you are, there's a next step that fits.
